Good Faith Estimate Notice

If you are not using insurance, you have the right to receive a Good Faith Estimate that outlines the expected cost of your care.

This is a federal requirement designed to help you understand and plan for expenses ahead of time. I can provide this estimate in writing at least one business day before services, and you’re always welcome to request one before scheduling.

If your final bill ends up being $400 or more above the estimate, you have the right to dispute the charges.

Be sure to keep a copy of your estimate for your records.

You can learn more about your rights at cms.gov/nosurprises, or by contacting the Colorado Division of Insurance at 303-894-7490 or 1-800-930-3745.
